Living Trust
This is your “legal box”–It holds all of your important assets and contains instructions regarding the management and distribution of them throughout your life and even after your death.

Pour Over Will/Last Will
This is a will that “pours over” assets into your trust that have not been titled in the name of your trust during your lifetime. (A “catch all” for loose ends).  It also revokes any “old” wills you may have executed in the past.

Power of Attorney for Health Care And Hippa Release and Authorization
This document appoints the individuals you choose to make health care decisions on your behalf, any time you are unable to make health care decisions for yourself and the HIPPA release document allows healthcare providers to release health information and/or medical records to any agents, trustees, and/or successor trustees appointed by you.

Power of Attorney for Non-Trust Matters
This document appoints the individuals you choose to handle your non-trust assets and financial affairs on your behalf, should you become unable to do so yourself.

Affidavit of Trust
This is proof you have a trust.  This document is utilized to re-title assets from your name individually to your name as trustee of your living trust.  (Puts assets into your legal box).

Assignment of Personal Property
This document assigns all your personal effects such as furniture, appliances, jewelry, collections, etc., into your living trust.

Personal Effects Memorandum

This document allows you to designate certain personal items or heirlooms to be distributed to specific individuals upon your death.
